Louisiana
The Louisiana Department of Health, Office of Public Health, publishes the cross-connection rule and the lists used for backflow work. The 2016 final rule says backflow preventers shall be tested by a State Plumbing Board of Louisiana licensed plumber who holds a water supply protection specialist endorsement, or by a tester who meets ASSE 5110-2009, or by a person holding a testing certificate from a nationally recognized backflow certification organization approved by the state health officer. Landscape-irrigation backflow preventers may be tested by a Horticulture Commission licensed landscape irrigation contractor who holds the plumbing board’s special water supply protection specialist endorsement. The Department posts an approved general-tester list.

Installation, repair, and maintenance follow a similar split. The rule says those tasks shall be done by a plumbing-board licensed plumber with the water supply protection specialist endorsement, with a parallel path for a licensed landscape irrigation contractor who holds the special endorsement. Preventers on public property, or otherwise under the complete control of the water supplier, may be installed, repaired, or maintained by a repairer who meets ASSE 5130-2009 or who holds a repairer certificate from a nationally recognized organization approved by the state health officer. The general-tester application asks for the certifying body, certificate number, issue date, and expiration date.
The Bureau of Engineering Services page points to the rule, the plumbing-board roster of plumbers with the water supply protection specialist endorsement, the approved general-tester list, and the approved cross-connection surveyor materials. The general-tester PDF is a Department list of names and companies. It is a public roster, and the page says licensed plumbers with the endorsement are certified to install, repair, and test. A customer should match the person on the job to the path in the rule: plumbing-board endorsement, ASSE 5110-2009, another approved national certificate, or the irrigation endorsement where that path applies.
The application form in the Department’s file is addressed to Engineering Services in Baton Rouge and lists an office telephone of 225-342-7471 for the compliance engineer named on the form, plus a bureau telephone of 225-342-7499 on the form footer. Those numbers are printed on the form reviewed. They are contacts for the application, not a statement of a test fee. The rule text used here does not set a dollar fee for the test itself. The water supplier still decides how the customer submits the passed or failed report.
